Serving seniors across multiple Midwestern states and numerous communities, both large and small, requires collaboration on all fronts. Over this last fiscal year, the Christian Horizons senior living and support services team navigated through the sometimes turbulent waters of our nation and economy to keep the safety and wellbeing of those we serve steadfastly at the forefront.
Teams across our senior living communities and older adult services—from CNAs and RNs to housekeepers and support associates—came together to lift up our mission and serve together as One CH.
OneCH – coming together as one – was essential to our focus for fiscal year 2022 (July 2021 through June 2022). We know that we are stronger together and saw this OneCH approach through every aspect of our work this past year, including:
- A recommitment to the work we do, including a strong focus on short- and long-term viability of our services.
- A reimagining, and sometimes repositioning, of our services and offerings to meet a new generation of older adults and associates.
- New ways of reaching older adults, including opening the doors of New Horizons PACE, an innovative approach to support in St. Louis.
We’re proud of this last fiscal year, our accomplishments, and the incredible heart of associates across our communities and services. Throughout our life plan communities, independent living options, assisted living communities and more, we lead with heart and joy to support community seniors.

Fiscal Year 2022 Annual Report
As a faith-led, person-first organization, Christian Horizons is in service to a mission of empowering older adults in mind, body and spirit through faith-led service. Based in St. Louis, Mo., the organization owns and operates a portfolio of seven life plan campuses and five stand-alone older adult communities offering a mix of independent, assisted and supportive living; memory support; long-term healthcare centers and short-term rehabilitation. The organization also serves older adults through Safe Haven Hospice in central Illinois; Senior Care Pharmacy Services; and New Horizons PACE® (Program of All-Inclusive Care for the Elderly) in St. Louis, Mo.
